NCPA members and other pharmacy interoperability stakeholders are invited to review and provide feedback on a set of interoperability use cases drafted by the Interoperability Matters Pharmacy Work Group operated by the Sequoia Project. The four use cases are 1) chronic disease management, 2) hospital discharge to home, 3) point-of-care testing, and 4) closing gaps in care.
