Things to remember if you need DMEPOS accreditation or relevant exemption

NCPA January 13, 2026

If you need DMEPOS accreditation or a relevant exemption in the wake of the recent DMEPOS final rule requiring DMEPOS suppliers to be surveyed and reaccredited every year (as opposed to the current 3-year cycle), you need to keep a few things in mind.

To supply DMEPOS, all pharmacies must get accredited by a CMS-approved national accreditation organization (AO) or get an exemption from the national provider enrollment (NPE) contractors.

Additionally, pharmacies may file an accreditation exemption statement that allows them to be enrolled in Medicare to supply DMEPOS that require accreditation if they meet these criteria:

1. Your DMEPOS billing is less than 5% of pharmacy sales the past 3 years;

2. You've had DMEPOS-enrolled supplier status and a provider number the past 5 years;

3. You've had no final adverse actions the past 5 years; and

4. You signed and submitted an accreditation exemption attestation to your NPE contractor proving you meet the first 3 criteria and agree to submit requested materials during an annual audit of randomly sampled pharmacies.

For details on DMEPOS accreditation, see CMS resources here and here.
