The Medicare Part D Medicare Drug Price Negotiation Program (MDPNP) is now underway as of Jan. 1. If you are filling prescriptions for Medicare Part D patients for any of the 10 negotiated drugs for 2026, you must be enrolled in the Medicare Transaction Facilitator (MTF) or you risk not being paid in full for these drugs. At the point of sale, you will be paid no more than the Maximum Fair Price (MFP) for the covered drugs. Approximately three weeks later, you should receive a "true-up" refund directly from the manufacturers. If you are not enrolled in the MTF, you are at risk of not receiving your manufacturer refunds, so please take action immediately! NCPA also recommends enrolling in Beacon MFP.
NCPA has been advocating on your behalf to make this transition as smooth as possible, and we will continue pushing for fair and timely payment for these drugs. It is our understanding that all manufacturers will be refunding pharmacies the Standard Default Refund Amount, defined as WAC minus MFP, for these products. Reconciliation will be essential. We are closely monitoring the rollout of this program and will promptly share any concerns with CMS.
