Getting to grips with records for direct patient care services

NCPA July 29, 2025

Effectively documenting direct patient care services is critical to being fairly reimbursed for them. Our Community-Based Pharmacy Patient Clinical Record Rubric sets an aspirational standard for community-based pharmacy patient record systems. Developed by the NCPA Innovation Center, with support from the Community Pharmacy Foundation, it outlines 46 must-have elements for strong, efficient, and clinically meaningful documentation. That can help you keep track of what you need to collect for a patient’s long-term record while evaluating the technology solutions that work best for you.
